Log in to the Cloudfon CX Panel.
In the left navigation bar, go to Live Chat Omnichannel Widget > My Widget. Select a Chat Plan Widget from the My Widget drop-down list,
Go to Web-chat.
Go to the Post-chat tab and turn on the Post-chat toggle key to enable your Post-chat window.

Once the new field is saved in the Fields section, you can click on the Edit icon to edit and the Remove icon to delete it.

In Field Layout, you can select Place text label above the input or on the left of the input.
